Full Job Description

This is a venture supporting clinicians & allied healthcare professionals across the four practices in Selby Town PCN.

To address the ever-increasing demand for primary care services there is a recognition of the need for better support for clinicians within practices whilst at the same time undertaking work in connection with Population Health Management. Selby Town PCN have a proven track record of developing new roles introduced as part of Primary Care Networks (PCNs) & this role is new to the PCN. Our locality network is committed to developing a multidisciplinary workforce which it sees as essential in providing a high level of care to its population., The GPA will support PCN staff, including the Dietician & Health & Wellbeing Coach in the smooth running of the services they provide. This may include performing the more routine administration and clinical tasks.

The award-winning Selby Town PCN consists of four practices with a combined population of approximately 50,000 patients, covering the large geographical area surrounding Selby and Escrick.

The PCN works over four practice sites (Beech Tree Surgery, Escrick Surgery, Posterngate Surgery and Scott Road Medical Centre) our local community hospital & The Summit, an ex indoor adventure park. The PCN has a Clinical Director, two Clinical Lead GPs and is managed by the PCN Operations Manager. We work in increasingly collaborative partnerships across a number of workstreams, which include important priority areas of mental health, frailty, urgent care and health inequity. Our team consists of a diverse multi-professional workforce which includes pharmacists, MSK practitioners, mental health workers, care coordinators, social prescribing link workers, a frailty therapy team, an urgent care coordinator, ANPs, paramedics, ACPs, a transformational lead, a digital lead, a dietitian, a health & wellbeing coach & an HR Lead.
