Social Care Practitioner - Front Door

15 to 37 hours per week.
Applicants should be aware that it is intended that this role, as part of the Statutory Services team, will transfer into RBWM (home | Royal Borough of Windsor and Maidenhead (rbwm.gov.uk)) in the coming year.
Optalis are looking for a Social Care Practitioner to provide comprehensive information and advice and signposting / connection to community based resources taking a strengths based approach to those who would require or meet the eligibility for Adult social are services / funding.

To deliver Statutory Services to people who meet the eligibility criteria of the Adult and Community Services Directorate, by engaging with the public to undertake assessment of their needs and arranging support in meeting their wellbeing and needs.

To maintain records of accountability for the service.
