Full Job Description

To be part of the annual review process for 1st and 2nd year students and ensure that it is an intrinsic part of students' EHCP annual reviews and college experience. This is a demanding and fulfilling role that requires effective liaison with college staff, students, families, and external stakeholders to shape a students' next steps.,

  • Be part of a person centred, welcoming, informative and professional team, supporting students and families through the 'annual review' process.

  • Manage your own caseload of 1st and 2nd year reviews in a timely manner, adhering to the legal framework.

  • Chair review meetings and complete all associated administrative work including local authority Educational Health & Care Plan paperwork & referrals to other agencies with guidance from the senior management team.

  • Ensure accurate 'annual review' data is maintained for college management team and contribute to case progress reports as appropriate

  • Liaise with external partners to provide comprehensive guidance and support for QAC students during and after their journey at QAC

  • Enter data onto the electronic transitions systems, including the college MIS system (Databridge)

  • Work collaboratively across the college to ensure positive outcomes for our students

  • Support the Transitions Team to include early discussions regarding transition pathways for all students from entry

  • Support the Transitions Team with internal and external events promoting the work of the transitions team

  • Share the College's commitment to safeguarding, ensuring training is up to date and helping to create a culture and environment where children and vulnerable adults' wishes and feelings are listened to and taken into account amongst staff, in any measures the College may put in place to protect them.

  • Obtain access to resources and attend any relevant or refresher training courses.

  • Encourage a culture of listening to children and vulnerable adults taking account of their wishes and feelings, among all staff, in any measures the College may put in place to protect them.

  • Perform such other duties commensurate with the status of the post as the senior management shall from time to time decide., You are required to upload a CV with details of your full employment history (since leaving school, including education, employment, and voluntary work) including reasons for any gaps in employment.
