Full Job Description

The post holder is responsible for providing clinical leadership within Connect18. Liaising with CAMHS and adult patients requiring transition across services. The leadership and development of service provision for young adults aged 18., The post holder is responsible for delivering a model of transition, intervention formulation and treatment and supervision as per evidenced based practice. This being held within the Connect18 transition service, and across CAMHS.
The post holder will work autonomously within professional guidelines and exercise responsibility for their practice. They will contribute to policy changes within the areas supported by the service and assess the needs of service users, suggesting areas for development.
