Palliative Care Social Worker

We are looking for a palliative care social worker to specialise in working with adults and children who are at the end of their life, their families, those they are close to and their communities.

The post holder will be part of a small social work team and use their particular skills and knowledge to help people to deal with the impact of what is happening to them.

This will include loss and bereavement, discharge planning, assessments and to have a good life and a good death. They will work in partnership with people they offer support to and work alongside other professions, agencies, organisations and as a member of the wider palliative care multi-disciplinary team.

The hospice's vision is to provide excellence in specialist palliative care and to ensure patients receive consistently high standards of care in all care settings within the organisation. There is a culture of reflection, training, supporting and development for staff, with access to regular clinical and management supervision.
